Once you become a client, information related to your care may be considered Protected Health Information under HIPAA and other applicable privacy laws. Continuing Care Counseling maintains a separate Notice of Privacy Practices that explains how clinical information may be used and disclosed, your privacy rights, and the practice’s legal duties.
You have rights regarding your health information, including the right to request a copy of your records, request corrections, request confidential communication, ask for certain restrictions, and file a complaint if you believe your privacy rights have been violated. HHS’s model Notice of Privacy Practices describes these rights and provider responsibilities.
Emergencies and Crisis Situations

Billing & No Surprises Act Notice
Billing & No Surprises Act Notice
Continuing Care Counseling is committed to clear, transparent billing practices. This notice explains general billing policies, insurance and private pay options, and your rights under the federal No Surprises Act.
Fees and Payment
The private pay fee for therapy services is:
Individual therapy session: $175 per session
Payment is due at the time of service unless other arrangements have been made in advance.
Continuing Care Counseling may accept payment by credit card, debit card, HSA/FSA card, or other approved payment methods. Clients are responsible for keeping payment information current.
Insurance Billing
Continuing Care Counseling works with insurance through Headway. If you choose to use insurance, your insurance benefits, eligibility, copay, coinsurance, deductible, and claims may be managed or verified through Headway and/or your insurance company.
Using insurance does not guarantee that services will be covered. You are responsible for understanding your insurance benefits, including deductible amounts, copays, coinsurance, prior authorization requirements, and any coverage limitations.
If your insurance denies a claim, applies the session to your deductible, or determines that you are responsible for part or all of the cost, you may be responsible for the balance allowed under your insurance plan and applicable law.
Private Pay
You may choose not to use insurance and instead pay privately. Some clients choose private pay for reasons related to privacy, flexibility, diagnosis concerns, or insurance limitations.
If you are uninsured or choose not to use insurance, you have the right to receive a Good Faith Estimate of expected charges under the No Surprises Act. CMS states that providers and facilities must give uninsured or self-pay individuals good-faith estimates of expected charges for scheduled health care services.
Good Faith Estimate Notice
Under the No Surprises Act, you have the right to receive a Good Faith Estimate that explains how much your care is expected to cost if you are uninsured or choose not to use insurance.
A Good Faith Estimate will include the expected cost of scheduled services with Continuing Care Counseling. For ongoing therapy, the total cost depends on the frequency and number of sessions you attend. You are not required to attend a set number of sessions, and you may discontinue therapy at any time.
CMS states that if you schedule care at least 3 business days in advance and are not using insurance, you generally have the right to receive a Good Faith Estimate. If care is scheduled 3–9 business days in advance, the estimate should be provided within 1 business day; if scheduled 10 or more business days in advance, it should be provided within 3 business days.
You may also request a Good Faith Estimate before scheduling services.
Example Estimate for Private Pay Therapy
The following is an example only. Your actual costs depend on your treatment plan, session frequency, and how long you choose to continue therapy.
Service: Individual psychotherapy
Fee: $175 per session
Estimated cost examples:
1 session: $175
4 sessions: $700
12 sessions: $2,100
24 sessions: $4,200
52 weekly sessions: $9,100
This estimate does not require you to attend any specific number of sessions. Therapy frequency and duration are determined collaboratively based on your needs, goals, preferences, and clinical appropriateness.
Changes to Estimates
A Good Faith Estimate is not a contract. It is an estimate based on information known at the time it is created.
If your treatment needs, session frequency, fees, or services change, Continuing Care Counseling may provide an updated Good Faith Estimate. Federal regulations require a new Good Faith Estimate if there are anticipated changes to the expected charges, services, frequency, duration, providers, or facilities.
If Your Bill Is Higher Than Your Estimate
If you receive a bill that is at least $400 more than your Good Faith Estimate, you may have the right to dispute the bill through the federal patient-provider dispute resolution process. CMS describes this process as available to uninsured or self-pay consumers whose bill is at least $400 more than the expected charges on the Good Faith Estimate.
You may also contact Continuing Care Counseling directly to ask questions about your bill or request clarification.
Balance Billing and Surprise Billing
“Balance billing” generally refers to being billed for the difference between what a provider charges and what an insurance plan pays. The Minnesota Department of Health explains that surprise billing can happen when a person cannot control who is involved in their care, such as emergency care or certain out-of-network services at an in-network facility.
